The DFCM’s Role within the Durham OHT
The (DFCM ) at Lakeridge Health plays a pivotal role within the Durham OHT, particularly through its involvement in the Primary Care Network (PCN). As part of this collaborative framework, the DFCM acts as a bridge between hospital care and community-based care, helping to strengthen communication and coordination across the continuum of services.
Through this partnership, the DFCM supports the Durham OHT’s vision of integrated, patient-centered care by ensuring that family physicians and primary care providers are well-connected to the broader health system. By participating in the OHT and PCN, DFCM members contribute to the development of a health–care model where primary care providers are central to patients’ overall care, with access to a wider range of allied health services and specialists. This integrative approach aligns with the DFCM’s goal of enhancing system capacity, equity, and performance across Durham Region.
