Durham region is fortunate to have an array of experts covering multiple topics and viewing the patient journey all from unique perspectives.  Some may focus on the socioeconomic background of the patient, others supporting caregivers and families during a patient’s acute health crisis.  All wanting physical, social and emotional health and wellness for the patient.  It is important for all of primary care to be aware and educated regarding all regional supports and services essential for our patients.
